Line numbers unaligned when ToC de-activated

When switching off the ToC manually (we have set disable_by_default: false as described in #30) then the line numbers are not any more in line with the text.

Still an issue. Not reproducible on my side (glitch?).

@rasos did you try it in isolation away from other plugins / css changes? :) It looks like maybe something else was involved..
