move remaining issues from bitbucket repo

The remaining open issues in the bitbucket repository should be moved to the repo's issues list. These are still open: https://bitbucket.org/eschnett/cactusamrex/issues?status=new&status=open&is_spam=!spam

I manually copied over the two issues that relate to CarpetX (and not hydro or spacetime codes).

A full copy of the issues can be exported from https://bitbucket.org/eschnett/cactusamrex/admin/issues/import-export and I have stored a copy which could potentially be used with GitHub's API to try and recreate them on GitHub. This would be quite a bi of work though.
