Update Qt to use Qt5 API

Currently we adapt Qt5 code back to a Qt4-ish API, but there are rough edges. Along with #510 we should modernise the API that is used.

This should probably include deprecating "qt4" packages and renaming as simply "qt", although that may need to wait for the next major release.
