lsp-headline-breadcrumb-mode is use material icons (which I don't have installed in system)

seems to be missed in installation section

Are you referring to lsp-headerline-breadcrumb-mode? I don't think lsp-ui customizes the headerline hence this issue should belongs to lsp-mode. See lsp-mode/lsp-headerline.el

Installing missing fonts via https://github.com/domtronn/all-the-icons.el fixes this for me.
