I think this error can also happen 0.23 or later when drinking mutation, because of mutation having a prompt (https://github.com/crawl/crawl/commit/b30acc428711). This case should be fixed by https://github.com/elliptic/qw/pull/28/commits/2b08a569c6488be2336ac8d8f984833463f9da36 in #28.
