Add Rule Documentation

This pull request adds docs for all rules except no-complex-switch-case.

Descriptions and examples were adapted from Elijah Manor's talk on Code Smells at Code on the Beach 2015, as well as a couple I put together.

It also adds rule descriptions to README.md and corrects inaccurate @fileoverview notes in rule and test files.
