You could add the feature of being able to send voice message, video. Audio filters can be created to mask voice messages so that the person's voice cannot be recognized.
Possibility of sending a photo or video with the webcam.

I'm curious also whether matrix.org presence schema supports geolocation metadata, or is it extensible to add as custom metadata?
The first option is that the matrix server supports geolocation metadata. We use geolocation a lot to create fire or disaster warning maps.
