Add GitHub issue and PR templates

Description

This PR makes the following changes:

  • [x] Add pull request template
  • [x] Add feature request and bug issue templates

These templates will make it easier for contributors to submit issues and pull requests by communicating what information we would like.

Please consider these a draft! We can tweak them to fit the project.
