Support for subqueries with lazily loaded shapes

• Subqueries aren't currently restored from disk on startup anyway, so there's no need to think about lazily loading them.
• Need to ensure that subqueries touching existing but not active shapes (no running consumer) will work as now -- the consumer should register itself as now.
• Lazily instantiated subqueries (including materializer) need investigation...
