When running on GNOME with fractional scaling on Wayland, Quake3e runs lower resolution, and GNOME scales it up from 1440p to 2160p resulting in a blurry image. Every time I play, I have to set display scaling to 100% in GNOME display settings.
You might be missing SDL_WINDOW_ALLOW_HIGHDPI in the window creation, or you might be scaling based on GNOME's request.
To enable fractional scaling in GNOME gsettings set org.gnome.mutter experimental-features "['scale-monitor-framebuffer']"
