Re-structure documentation

The README.md file is too long, makes it difficult to read or look up information, also to organise and edit, there are even duplicates. Therefore I have moved the main guides into separate documents so that we can organise them easier, including correction of Markdown syntax and indentation.

I know this is a huge refactor, tried my best to migrate and improve the documents.
