dotnet-debugger-extensions doesn't have documented direct download links

The dotnet-debugger-extensions install instructions don't list any direct download URLs which makes it harder for users to acquire the tool. I'm not sure if this is just a missing doc update or if there is unfinished work to host single-file versions of the app.
