UI - Favorite Page: dialog redesign

As an user I need to redesign the Favorite Page dialog to meet new design for UX

currently it shows everything on one column

New design needs it to be on two columns, and the "Preview" thumbnail needs to be placed on the right column, and all the input fields on the left column
