Supports copy instructions for media files

copy instructions for media files can be created on request be the stylesheets. Works on with "chunked output", because otherwise they don't know the destination (root of HTML output). When the file:copy function is available, they can perform the copy operations.

New section 3.4.1 in "Managing media" explains the details.
