Footnote styling

Footnote superscripts are sort of indistinguishable from inline equations with exponents.

Instapaper uses a little "..." bubble (as replicated by this library). It would mean the footnotes couldn't be referenced by number, but perhaps that's okay if the footnotes reference link back to an anchor in the text. In print, numbers could be used.

One reader reports that it took them a while to realize they could hover on footnotes. We should probably do something to visually queue it a bit more.
