Add missing html5 elements and disabled style

I don't fully agree with the opacity change on disabled elements. Reason for my disagreement is that .5 opacity set on #222222 on plain white is the equivalent of #888888 on plain white. This fails both WCAG AA and WCAG AAA tests with some margin (you can try here). Especially when Skeleton has a very thin default font.

That said, some sort of indication that elements are disabled would be nice.
