IDOv2.0 should probably qualify as a supported mod now
I uploaded my mod at https://www.moddb.com/mods/infernal-death-orgy-v20
It uses Dhewm3, as I played through all the levels without crashes.
Just a few little nit-picks; the following is copied-and-pasted from my IDO_readme (highly detailed).docx file, which is contained in the mod's download:
_8. I think this is a bug with vanilla Doom 3, but when you run out of grenades, the weapons indicator in the lower-right of the HUD doesn’t remove the dot or number associated with grenades.

I think this is also a bug with vanilla Doom 3, but you grunt in pain when you are running out of oxygen. Couldn’t figure out how to fix this.

I think this is a bug with Dhewm3 release candidate. If I skip some monster introduction cutscene by pressing Esc, the cutscene audio continues._
1.5.1 RC3 is out, and the mod dlls include an updated one for dentonmod - could you test if it works with your mod? https://github.com/dhewm/dhewm3/releases/tag/1.5.1_RC3
no idea about 8. and 9., must be a bug in the scripts or something (possibly also in the main game); fixing scripts is outside the scope of dhewm3 10. is a known bug I haven't gotten around to fix yet
Sure, I think I can complete a full run through of IDO with DOOM 3's basic levels by the end of next Sunday.

Please pardon the delay. I have successfully played my Infernal Death Orgy v2 mod with your new RC 3 of Dhewm3. The only little (potentially new) glitch I have noticed is that recording or streaming Dhewm3 RC3 with OBS Studio strangely flips the screen upside-down and left-to-right, as you can see at https://www.twitch.tv/videos/939757223 (skip to 1-minute time point).
Take into account my machine specs: CPU: Ryzen 5 3600 (2019) GPU: Radeon RX 5700 (2019) with 20.11.2 driver (latest official release) Memory: 16gb DDR4 3200
In addition, I have petitioned a few friends to try out my mod + RC3 on their systems, one of which uses a modern Nvidia GeForce GPU. If they experience success or glitches, I'll let you know. But from my perspective, RC3 seems pretty good!

Most probably a bug in OBS Studio (otherwise in AMDs driver), but you could try if using dhewm3_notools.exe instead of dhewm3.exe helps
