If titles are not meant to be directly in-sync with the leaf level of the hierarchy, performing a refactor can cause loss of desired title text.

Add a config to toggle whether titles are touched during refactor command.

Quickly trying to spec this one out more - is it fair to say that you would only want titles that you've manually edited to remain intact, and ones that you haven't touched to change with the migration?
Wondering if it makes sense to make this an explicit setting or to automatically pick the behavior per-note based on whether the title had been modified before.
That's a great question!
I personally would be most comfortable with some global config setting I can toggle on/off as need be (it would probably always be off).
That said, detecting manual title changes may benefit others.
