Can the firmware be modified to make this a MIDI controller, sending MIDI over USB instead of keypresses?

It "could", someone needs to change the USB class type and change the keycodes sent to the host.

But someone has to do the work :)

Not officially at the moment, but some helpful users have ported QMK to duckyPad, and maybe that supports MIDI?
