Review old example notebooks

Update these, or eliminate if no longer needed. Want to remove possibility for confusion. Use sample data from #7 in these examples.

Also remove glm_lma_param_space.sh, and update docs/index.rst to indicate it's not there.

Alternately, update that script to show use after fixing #13 .
