RFC 0453 - missing transition from Receive request-credential to Send problem-report

I know that the state diagram illustrates the happy path but as per 0453/0511 the interaction can also start with a request-credential message from the Holder to the Issuer it is probably worth mentioning that the Issuer could reject the request ("Reject Request") with a problem-report.

@swcurran @llorllale should we add that transition to the state machine in 0453?

@awoie what is missing is a state machine table (eg. did-exchange) that would comprehensively answer these questions.
