Expose as a express middleware

Looks really neat, but I could not find any information about why is this not exposed as a express middleware? This would make it follow the best practice middleware.

@kevinsimper it's a legacy holdover from the original implementation, and to this point nobody has really looked into converting it over to be a middleware. Perhaps you'd be interested in taking a look? I'm available to help with any questions. One concern I do have is that it might be slightly more difficult to maintain epilogue's support for both express and restify this way?

+1 since it would also allow the use of epilogue with Koa!
