Example bookdown footers don't appear correctly

I've been struggling to troubleshoot issues with why the custom template header and footers don't appear correctly in my output document. I went back and ran the example bookdown that comes with the package and noticed the page numbers don't always appear. In my case only page 10 is numbered.
