Remove {{extract image}} from new uploads

The {{extract image}} template, which marks that images need to be extracted from a file, should be removed from the page content of the new page when uploading the crop as a new image, as the extraction has already been done.

It can be left on the source page, because it's possible that further images need to be extracted. Perhaps an option could be presented to remove the template from the source page as well.
