moodle-local_recompletion notification setting to determine who gets recompletion message

Add a new notification setting to determine which user gets a recompletion notification. Original enable/disable email notification stays the same, but the following additional 'levels' are added:

Send to all users (with a completion record) - If a course completion record exists for a user, they will be notified.
Send to all users (with an enrollment) - If a user is enrolled (active, suspended, etc) in the course, they will be notified.
Send to all users (with an active enrollment) - If a user is actively enrolled, they will be notified.

can't really give a specific timeframe as I have to fit this in as a volunteer - it's pretty high on my list though so hopefully in the next few weeks or so but feel free to keep bugging me if you don't hear anything from me :-)

I've just squashed and rebased this one into #170 - will try to get some testing done on that shortly so we can finally merge it! - thanks for your patience! :-)
