Switch replacement to the new insertion model

Currently replacement utilizes the old insertion model.

• [ ] Migrate deletions to the new insertion model
• [ ] Migrate replacement actions to use SelectionBoundary, rather than ContentLocations

Doing this after #70 makes more sense, since deletion will have changed interfaces defining the actions.
