not wotking in 1.9.2

thanks for the code but its not working. I added it in my custom theme folder but still... i cannot make it work.. please let me know if any suggestions. P:S i see no changes on frontend and no new module in admin side.

Is it working with the default or rwd theme?

I have not tried it on default or rwd.. Our website is using its own custom built theme .
