Support for a-la-gitlab "Discussions" in Patch Proposals

In Git(Lab|Hub), users can start multiple separate discussions on a MR/PR and each of those discussions has a "Resolved?" status. This can be used to track how the review process is going and is even used as one of the "Checks" on an MR/PR: all discussions need to be resolved before a MR/PR is green for merging.

This is a tracker issue (for the time being), to cover adding this functionality inside the plugin.
