First focused element

Determine whether an initial focus elsewhere than on the first focusable element in the DOM is tested for by any package and, if not, add a test for it.

alfa rule r87 and qualWeb rule QW-ACT-R72 require the first focusable element to be a link to non-repeated or main content. But there is no rule requiring the element that is initially focused to be the first focusable element.
