The wiki says "you need to create a new folder in the mods directory of you minecraft installation" but never says what to name the folder. That info should be on the wiki (probably within the quoted sentence). [This is on the getting started page]

The folder can be called as you like, within reason. This is a replacement archive fashion. The ability to create a mod using Custom Stuff 4 only on the Forge 2655 version. The mod itself can work on the latest versions.
