Add name to workflow history web UI

Quick and dirty fix for #136:

The description says scheduled activities, but we might also want to show it for completed ones too? That might be more complex though since it's not in the attributes.

Also, I filtered out the workflow name since that's sort of redundant there but it's easy to add back if it makes it more consistent.
