More size compatibility

Would be nice if we could have options for 3x5, 4x6, 5x8 card sizes (standard US index card sizes). I don't know about anyone else, but both my printers print on 3x5 note cards and larger.

There's already an issue for adding more standard sizes: #25. In addition, it would be nice if the user could choose a custom card size.
