Add ArtifactPackageProcessor

The artifact packaging logic could be improved by:

  1. Abstracting so that the packaging logic happens for "artifacts", rather than specific types of artifacts as it is now
  2. Refactoring so that the packaging logic happens exclusively at the level of FHIR objects, rather than relying on the IO layer directly
