Migrate queryPattern functionality from the ruler to the evaluator

The cqf-ruler implementation of $data-requirements will return DataRequirements with a fhirQueryPattern extension. This functionality should be available in the DataRequirementsProcessor directly so it can be surfaced in the evaluator as well.
