Setup the Evaluator/Clinical Reasoning repository

Acceptance Criteria: The Clinical Reasoning Repo is migrated to the cqframework organization, all developers are made aware of this, and the following tasks are completed

• [ ] Add in-memory implementation of FhirRepository for testing
