If evaluation fails, the entire operation should return an operationoutcome If evaluation succeeds, then the MeasureReport should contain a contained OperationOutcome with any warnings/messages generated by the evaluation, consistent with the proposed specification change here: https://jira.hl7.org/browse/FHIR-37774
