Breakout Patient Registration Form into Patient Registration and Patient Health Profile Forms
To improve the clarity and organization of patient data, the existing patient registration form in the CARE system needs to be split into two distinct forms:
- Patient Registration Form: This will capture basic patient details.
- Patient Health Profile Form: This will capture detailed medical history and health-related information.

- Address (add a short text field for "Landmark")
- Emergency Phone Number and Name (new)
Social Profile

- Socio-economic Status (new):
- Well off: treatment and food
- Middle class: costly treatment - difficult, food - okay
- Poor: basic treatment - difficult, food - okay
- Very poor: food support needed
The description for socio-economic status options can be shown in the UI as a hint on hover of (i).

We dont really need a separate table for this right now, we can just keep everything in the same table and use the same API for multiple forms. a PATCH request with the same endpoint will only update what is needed.
@gigincg when is this expected? Dr. Athul needs this to start getting IAPC to actively scale. Please provide some timeline
