Medicine administration record: Withhold

Add feature to withhold a medicine in the administration record and well as prescription.

The withholding will be marked with a reason (long text box) and for a specific time period (in days)

The withheld medicine will appear in the administration page as greyed out.

As per discussions,

We could add a DateTime Field to specify when it is to be re-continued.

However, moved to the wishlist until we build more depth on this.
