Multiple beds "in use" by same patient

Describe the bug

• Able to assign multiple ConsultationBed records that are active for the same patient.
• This leads to another issue, now the patient cannot be shifted to a new bed either.

Tip: My mouse has a nice flaw these days, 50% of the time it accidentally performs double-clicks when a single click was intended.

To Reproduce Steps to reproduce the behavior:

1. Go to http://localhost:4000/facility/015ad911-39b7-4104-b304-bf8178c33830/patient/76742faa-30dd-4eb9-9649-fe718f947ec0/consultation/2ad89b7c-7bff-4035-a3c6-36d53551a8f0
2. See bed history

Expected behavior

• Multiple active ConsultationBed for the same consultation should not be able to be created. (DB Constraint)
• Should be able to shift for such buggy cases.

this issue is similar to the duplicate consultation issue, it is caused by a race condition
