Fixed presence-customcmd-setup to work with new hubs UI

Hubs has undergone a UI update that caused presence-customcmd-setup.js to stop working as intended since the location and names of a bunch of things in the HTML were changed. I've updated the code to adjust for these changes so that custom commands will once again work as intended.

Also, your documentation and code has helped me so much in trying to customize my own instance of hubs. Thank you :)
