Input Field Limits For Change Address and Index

To avoid the loss of funds, the Change Address and Index Input Fields:

Change Address values should be limited to a range: min="0" max="1" AND Index values should be limited to a range: min="0"

NOTE: I'm looking into the max value for the Index.
