fix: use process.env.VSCODE_PROXY_URI

Warning This requires changes to the Tabnine server before it will work correctly.

This adds a new util function called getExternalUri which checks for process.env.VSCODE_PROXY_URI and uses that with the external uri so that Tabnine can work in environments like code-server, Codespaces, Gitpod.

Seems like this could fix the issue with TabNine and similar extensions that need to talk to the code-server host on specific ports. Looking forward to seeing this completed.
