[SME] In 'medical expenses' sub-section, show uneditable row of already reported medical expenses

Better than saying 'don't repeat previously reported expenses'?

Shows up when there's a disabled head or spouse.

In some discussions, we agreed this could be a useful way to be more clear about what information shouldn't be repeated. It ended up that I did some incidental work on this in #695. Not sure if we'll wait till that one is worked out, or if we should work on this separately, but as far as a decision, this feature seems to be at 'go for launch' at the moment.

Part of the problem, though, is figuring out where expenses are reported and in what order to ask about expenses. Maybe we need to break down other sections into more specific questions. For example, for child care, maybe we need to include a question on medical expenses specifically. On the other hand, it may be important that those medical expenses are in the 'medical expenses section'. We may need a subject matter expert to tell us which section those would belong in, or if they would belong in both. Lots of questions!

It did actually bring up new questions about the calculations themselves. For example, does handicapped assistance get put in dependent care as well?
