Convert Setext style headings to ATX style ones

Current Setext style only allows two-level headings; too many <h1> and <h2> doesn’t look good on GitHub. GitHub’s CSS was designed for making one <h1> as title, bunch of <h2> for section, more <h3> for subsection, etc.
