breaks Q selection screen

after i installed g64 and libsm64 onto garry's mod, sometimes the Q selection menu thing just stops working. i press a button, it makes the sound and button animation but nothing spawns. and when I press Z to remove that item, it just removes the objects that I already spawned before.
