self.app should be available in init_app as well, otherwise it will fail when calling source function.

I'm wanting to use this package at work and am hitting the same bug. I can confirm that this pr fixes the error I'm getting. I know this pr is two years old, but if there's any way it could be merged and pypi updated, it'd be a great benefit to a good number of people (as searching Flask Google analytics pulls this project up as the first result).
