Duplicates error only shows items of last duplication

On the keyword screen citation-file-format github io_cffinit_ (2)

I expected to see an error message like must NOT have duplicate items (items ##0 and 2 and items ## 3 and 1 and are identical), but it says must NOT have duplicate items (items ## 3 and 1 are identical). The foo duplication is not being reported.

When I fix the duplication of bar then the correct must NOT have duplicate items (items ## 2 and 0 are identical) message appears.

The same happens on the identifiers and authors screens.
