Add a CONTRIBUTING doc

I'm thinking that as the project grows and folks want to help out, adding a contributing doc would be useful. I'm sure you've seen plenty of these, but here's an example of what my company is using for our docs repo: https://github.com/sensu/sensu-docs/blob/master/CONTRIBUTING.md. May be of use?

The Sensu doc is a good example. Obviously, we don't have testing bits (yet) but, it can almost be used 1:1. If you would like to take this on I would happily accept the PR!
