Links in the ReadMe are Outdated

Is your feature request related to a problem? Please describe. Links throughout the Readme.md file are outdated, pointing to the previous Chocolatey docs link, the Chocolatey GitHub wiki, and various other things. In addition, it mentioned Gitter, which is no longer in use.

Describe the solution you'd like The file (and others with URLs in) should be checked over and adjusted if there is a more up-to-date link available.
