Incremental mode in affiliate command

When doing affiliations, just work by default with identities who don't have an affiliation already.

When working on this, please have in mind that when moving to another organization, you may change the email address to be used.

This would lead to a situation where that developer is affiliated, but that developer would need a new affiliation.
